Our Office

Emergency Care:

In an extreme emergency dial 911.
If you have an urgent problem, please call our office for instructions. After hours, a physician will be on call for urgent situations. In the event an after-hours call must be attended to, there is a $25 afterhours service fee.

Practice Hours:

Our hours are Monday – Friday 8 AM – 4:30 PM. Laboratory services start at 7:30 AM.

Appointments:

Patients are seen Monday through Friday by appointment. We ask that you call in advance in order to schedule a convenient time. Every effort will be made to keep appointment commitments. Whenever possible you will be notified if there is a delay. If you cannot keep your appointment, we ask that you notify us as soon as possible at 407-345-0005. In the event that 24 hours’ notice of cancellation is not provided, there is a $25 fee for general appointments, and a $50 fee for missed physicals.

Prescription Refills:

We ask that all prescription refills be requested at the time of your office visit. Please assess any refill needs prior to coming to the office. We will gladly provide you with refills for up to one year or your next office visit. Please allow 48 hours for your request to be processed.

Telephone Calls:

Please feel free to call our office if you have any questions regarding your condition, medication or treatment. Our office staff can also assist you with questions regarding appointments. We ask that you always spell your name when leaving messages and give your date of birth. Please remember our physicians and medical assistants are treating patients during the day – we value your concerns – and will get back to you at our earliest convenience, which may be at the end of the working day.

Referrals:

Please feel free to call our office if you have any questions. In some instances a referral is needed for insurance and continuity of care. We ask that you call the office at least 72 hours in advance at 407-345-0005 ext. 103.

Patient Services:

We participate with many insurance companies. Please confirm with your insurance company that we are participating providers. We will collect your deductible, co-payment and fees for non-covered services, along with any balances at the time of your visit. We accept cash, checks, MasterCard, Visa, American Express and Discover Card. Please call our office at 407-345-0005.

Billing:

If you have questions about an invoice or insurance billing, please contact our billing department at 321-274-1862.

Medical Records:

Your medical records are completely private. To ensure patient confidentiality, and conforming to HIPAA regulations, no information regarding your condition will be released by this office to anyone, including any family member, without your written consent. If you have questions about medical records please call us at 407-345-0005 ext. 106.
